Patient care diary and selected aspects of the post-intensive care syndrome

Abstract:

Úvod do problematiky: Syndrom post-intenzivní péče, který je popisován u pacientů, kteří přežili kritický zdravotní stav, představuje významný problém, snižující kvalitu života těchto pacientů. V rámci prevence psychických obtíží je mimo jiné doporučováno vedení deníků péče o pacienta, jehož následné pročítání může pacientům ulevit od úzkostí, vysvětlit traumatizující střípkovité vzpomínky z hospitalizace, doplnit chybějící informace nebo pomoci uzavřít těžkou životní kapitolu. Cíl: Cílem diplomové práce je zhodnotit celkový vliv deníků na pacienty a jejich spokojenost s obdrženým deníkem péče. Empirická část práce má za cíl zhodnocení efektu deníků péče na navrácení ztracených vzpomínek, dále také identifikovat kognitivní a psychické potíže, kterými pacienti po propuštění trpí. Cílem práce je také získaná data využít k navržení změn v denících za účelem zefektivnění této intervence tak, aby pro pacienty představovala maximální přínos. Metodika: Kvalitativní data byla sbírána pomocí polostrukturovaných rozhovorů s bývalými pacienty Fakultní nemocnice Ostrava. Výzkumného šetření se zúčastnilo 11 participantů. Výsledky: Vytyčené cíle, které byly pro tuto práci stanoveny, byly splněny. U některých pacientů došlo při čtení deníku k obnovení vzpomínek. Nejčastějšími potížemi po hospitalizaci byly změny v emočním prožívání a obavy z možných opětovných obtíží. Všichni pacienti pozitivně hodnotili deníky; některým deník pomohl vypořádat se s těžkou situací či doplnit chybějící informace. Z rozhovorů vyplynuly návrhy změn ve vedení deníků, zejména snaha o zapojení rodinných příslušníků a zvolení osobnějšího přístupu. Závěr: Vedení deníků péče má na pacienty pozitivní vliv, avšak pro potvrzení efektivity deníků v prevenci syndromu post-intenzivní péče je nutné provedení rozsáhlejšího výzkumného šetření, ideálně kontrolované studie.

Abstract:

Introduction: The post-intensive care syndrome, observed in patients who have survived a critical health condition, represents a significant problem that diminishes the quality of life for these individuals. As part of the prevention of associated psychological difficulties, maintaining patient care diaries is recommended, with subsequent reading serving to alleviate anxiety, explain traumatic fragmented memories from hospitalization, fill in missing information, or assist in closing a challenging chapter of life. Objective: The aim of this thesis is to assess the overall impact of patient care diaries on patients and their satisfaction with the received care diary. The empirical part of the study seeks to evaluate the effect of care diaries on the retrieval of lost memories and to identify cognitive and psychological issues that patients experience after discharge. Additionally, the goal is to use the obtained data to propose changes in the diaries to enhance the effectiveness of this intervention for maximum benefit to patients. Methodology: Qualitative data were collected through semi-structured interviews with former patients of the Faculty Hospital Ostrava. Eleven participants took part in the research investigation. Results: The set objectives for this study were met, with some patients experiencing memory retrieval upon reading the diary. The most common post-hospitalization difficulties included changes in emotional experiences and concerns about potential complications. All patients positively evaluated the diaries; for some, the diary helped cope with challenging situations or supplement missing information. Suggestions for changes in diary management emerged from the interviews, particularly emphasizing the involvement of family members and choosing a more personalized approach. Conclusion: Patient care diaries have a positive impact on patients, but to confirm their effectiveness in preventing post-intensive care syndrome, further extensive research is necessary, ideally through a controlled study.
