Abstract:

Bakalářská práce se zabývá tématem zdravotně sociální péče o klienty, u nichž byla diagnostikována Alzheimerova choroba. Práce je rozdělena na část teoretickou a část aplikační. První tři kapitoly se věnují teoretickému vymezení demencí, popisují jednotlivá stádia Alzheimerovy choroby a role sociálního pracovníka při práci s danou cílovou skupinou. Dále práce popisuje možnosti péče o osoby s demencí v domácím i v institucionálním zařízení, včetně zdravotních a sociálních služeb. V aplikační části jsou uvedeny dvě případové kazuistiky, na kterých je demonstrováno, jak byla realizována péče o nemocné v praxi. Zjištěné poznatky jsou dále diskutovány v poslední kapitole.

Abstract:

This bachelor thesis deals with the health and social care of the clients who were diagnosed with the Alzheimer´s disease. The thesis is divided into two parts - theoretical and application. The first three chapters focus on the theoretical definition of dementias. Particular stages of the Alzheimer´s disease and the role of a social worker during the work with the target group are described in these chapters. Furthermore, the thesis describes options of taking care for the people with dementia in both a domestic and an institutional facility, including health and social services. There are two case studies mentioned in the application part, on which is demonstrated, how was realised care of clients in practice. These case studies are discussed in the last chapter.
