Specifics of care of a client with autism in terminal stage of the disease

Abstract:

Cíl: Cílem bakalářské práce bylo popsat specifika ošetřovatelské péče a přístup personálu v hospici u klienta s autismem a nádorovým onemocněním, u kterého byla lékařem doporučena paliativní péče. Metodika: Pro splnění stanovených cílů praktické části bakalářské práce byla vybrána forma kazuistiky. Klient byl vybrán záměrně jako zástupce osob s poruchou autistického spektra s onkologickou diagnózou bez možnosti kurativní terapie. Výsledky: Ošetřovatelská péče o klienta s autismem byla velmi specifická po celou dobu jeho hospitalizace, neboť se jeho chování značně lišilo od chování ostatních klientů. Často bylo výrazně složité s klientem navázat kontakt pro další spolupráci, protože projevoval silnou nedůvěru k personálu. Byla nutná intenzivní komunikace a spolupráce s rodinnými příslušníky. Závěr: Hospitalizace klienta s PAS bude vždy mírně komplikovaná projevy autistické poruchy. Veškerý personál se musí snažit o poskytnutí co nejlepší péče. Velmi důležitá je spolupráce multidisciplinárního týmu, aby se podařilo navázat kontakt a určitý funkční vztah, který je potřebný pro společné určení dalších možností péče. Nezbytná je snaha o vytvoření důvěrného vztahu mezi personálem a klientem. Je důležité, aby klient pochopil, že personálu může důvěřovat a je možné spolehnout se na jeho pomoc.

Abstract:

Objective: The aimofthebachelor thesis was to describethespecificsof care and approachofhospic'sstaff to a clientwithautisticspectrumdisorder and cancer, who has beenrecommendedfor paliative care byphysician. Methods: The paradigm of case studies was chosen to meet the designated goals of the practical part of the thesis. The client was deliberately chosen as a representative of people with autisticspectrum disorder with cancer diagnoses without curative treatment options. Results: The care for the client with autism was very specific throughout his hospitalization. His behaviour was very different from other clients. It has often been considerably difficult to establish contact with the client for further cooperation. He had a strong distrust of staff. An intensive communication and collaboration with family memberswas required. Conclusion: Hospitalization of client with PAS will always be slightly complicated because of manifestations of autistic disorder. All staff must strive to provide the best possible care. The collaboration of a multidisciplinary teamis very important in order to establish contact and a functional relationship that is necessary for joint assessmentof furthercare options. The effort to create trustful relationship between staff and clients is necessary. It is important that the client understands that he can trust the staff and he can rely on their help.
