Nutritional support for patients undergoing surgical treatment of colorectal cancer

Abstract:

Úvod: Kolorektální karcinom je nejčastějším karcinomem trávicího traktu. Jedinou kurativní modalitou je radikální chirurgická resekce karcinomu. Malnutrice chirurgických pacientů je spojena se zhoršeným hojením operačních ran, narušenou obranyschopností a sníženou svalovou sílou. V posledních letech se pozornost obrací k možnosti nutričně posílit předoperační fázi léčby kolorektálního karcinomu a tím zlepšit výsledky léčby. Cíle: Cílem disertační práce je zhodnocení vlivu předoperačního podávání orálních nutričních doplňků na soběstačnost, fyzický stav a nutriční stav pacientů, kteří podstoupili elektivní kolorektální resekci. Metodika: Prospektivní randomizovaná klinická studie byla provedena v FNO v období 1.1.2019 - 31.3.2020. Pacienti indikováni k operaci kolorektálního karcinomu byli randomizováni do 2 skupin. Skupina I užívala orální nutriční doplňky dvakrát denně po dobu 7 dnů před operací, skupina II neužívala orální nutriční doplňky. Sledovány, hodnoceny a analyzovány (v den nástupu k hospitalizaci a 5./6. pooperační den) byly demografická a klinická data, stav výživy (prealbumin a albuminu v krvi, obvod nedominantní paže, váhový úbytek za poslední období, NRS 2002), tělesná analýza (svalová hmota, tuková hmota a podíl vody v těle, svalová síla horních a dolních končetin), soběstačnost (index Barthelové), délka pobytu na JIP, délka hospitalizace a 30-ti denní pooperační komplikace (dle Clavien-Dinda). Výsledky: Do studie bylo zařazeno 120 pacientů (60 pacientů v studijní skupině a 60 pacientů v kontrolní skupině). Pacienti obou skupin měli srovnatelné výsledky ve sledovaných parametrech, bez statisticky významných rozdílů. Závěr: Předoperační podávání ONS pacientům podstupujícím elektivní kolorektální operaci neovlivnilo soběstačnost pacienta, fyzický stav a stav výživy. Podávání ONS nesnížilo ani pooperační morbiditu ani nezkrátilo pobyt pacientů v nemocnici.

Abstract:

Introduction: Colorectal cancer is the most common cancer of the digestive tract. The only curative modality is radical surgical resection. Malnutrition in surgical patients is associated with impaired healing of surgical wounds, impaired immunity and reduced muscle strength. In recent years, attention has been turned to the possibility of nutritionally strengthening the pre-operative phase of colorectal cancer treatment and thus improving treatment results. Aim: The aim of this dissertation was to evaluate the effect of preoperative administration of oral nutritional supplements on self-sufficiency, physical condition and nutritional status of patients who underwent elective colorectal resection. Methody: A prospective randomized clinical trial was conducted at FNO in the period 1/1/2019 - 31/3/2020. Patients indicated for colorectal cancer surgery were randomized into 2 groups. The group I took oral nutritional supplements twice a day for 7 days before surgery, the group II did not take oral nutritional supplements. Monitored, evaluated and analyzed (on the day of hospitalization and on the 5th/6th postoperative day) were demographic and clinical data, nutritional status (prealbumin and albumin in the blood, circumference of the non-dominant arm, weight loss during the last period, NRS 2002), physical analysis (muscle mass, fat mass and proportion of water in the body, muscle strength of the upper and lower limbs), self-sufficiency (Barthel index), length of stay in the ICU, length of hospitalization and 30-day postoperative complications (according to Clavien-Dindo). Results: 120 patients were included in the study (60 patients in the study group and 60 patients in the control group). Patients of both groups had comparable results in the monitored parameters, without statistically significant differences. Conclusion: Preoperative administration of ONS to patients undergoing elective colorectal surgery did not affect patient self-sufficiency, physical status, and nutritional status. Administration of ONS neither reduced postoperative morbidity nor shortened patients' hospital stay
