Nursing management in the dispensarization of patients with hypertension in the general practitioner ambulance

Abstract:

Diplomová práce je zaměřena na problematiku prevence a léčby arteriální hypertenze v oblasti primární péče. Arteriální hypertenze představuje významný rizikový faktor pro vznik kardiovaskulárního onemocnění a s ohledem na vysokou prevalenci v dospělé populaci představuje velký zdravotní i ekonomický problém. V teoretické části diplomové práce je definována diagnóza arteriální hypertenze, jsou popsány rizikové faktory vzniku, možnosti prevence a léčby hypertenze a správná technika měření krevního tlaku. Pozornost je věnována roli sestry v péči o hypertoniky. V praktické části jsou zjišťovány motivační faktory k léčbě z pohledu pacienta a z pohledu sestry. Cílem je zjistit rezervy v adherenci pacientů s arteriální hypertenzí k léčbě a režimovým opatřením a najít způsob, jak zlepšit adherenci pacientů. Na základě výzkumného šetření byly zjištěny rozdíly v motivaci pacientů k dodržování léčby arteriální hypertenze z pohledu sestry a z pohledu pacienta. Jako nejvíce problematická byla stanovena oblast dodržování zásad zdravého stravování, udržení přiměřené tělesné hmotnosti, dostatečné fyzické aktivity a vyhýbání se stresu.

Abstract:

Thesis is focused on the issue of prevention and cure of the arterial hypertension in primary care. The arterial hypertension is a significant risk factor for an origin of cardiovascular disease and according to high occurrence at adult population is a big health and economy problem. In a theoretical part of the thesis are described a diagnostic of the arterial hypertension, risk factors of an origin, a possibility of a prevention and a cure of the hypertension and a correct technique of a blood pressure measurement. The role of nurse in care of patients with hypertension is important. In a practical are found out motivating factors about the cure from patient and nurse sides. The target is finding out reserves in adherence to the cure of patients with the arterial hypertension and a regime action and finding out how to improve compliance of patients. According to the research there is a difference in patients motivation to observe of cure arterial hypertension from nurse side and patient side. As the most problematic area was determine observance principle of healthy eating, keep proportionate weight, enough physical activity and stress avoiding.
