Quality of life of elderly on dialysis

Abstract:

Goal: The main goal of the diploma thesis was to determine the quality of life and health satisfaction of dialysed elderly people in the individual aspects of life, with the purpose to find out how these patients evaluate key areas of their lives and how satisfied they are with them. Methodology: The research was conducted on 81 dialysed elderly patients who attended selected Dialysis center and nephrology ambulance in Slovakia from the 1. of April until the 30. of September. The data was collected in the mean of standardised questionnaire KDQOL-SF ? 1.3 (Hais et al. 1997) and evaluated based on the corresponded manual. Results: Based on the results of this research, it was found that the dialysed elderly had the highest life quality ranking in patient satisfaction, cognitive function, and quality of social interaction domains. The lowest life quality ranking was found in the burden of kidney disease, general health, role physical and energy and fatigue domains. A domain such as the effect of kidney disease, sleep, physical functioning, pain, and social function also reached lower scores. The research results also show that the quality of life of the dialysed elderly is not affected by the patient's sex or the dialysis treatment time. Treatment of dialysis therapy also does not impact the incidence of pain or the level of frustration in dialysed elderly. A significant correlation was found between sleep quality and the duration of the dialysis therapy and between age and vascular access problems in dialysed elderly. Conclusion: Dialysis therapy significantly affects the overall quality of life of the dialysed elderly. Therefore, it is very important that these patients are regularly monitored for quality of life, provided with highly individualized care, education, and support.

Abstract:

Cieľ: Hlavným cieľom diplomovej práce bolo zistiť kvalitu života a spokojnosť so zdravím dialyzovaných seniorov v jednotlivých životných oblastiach so zámerom zistiť, ako títo pacienti hodnotia kľúčové oblasti svojho života a ako sú s nimi spokojní. Metodika: Výskumný súbor bol tvorený 81 dialyzovanými seniormi dochádzajúcimi do vybraného Dialyzačného centra a nefrologickej ambulancie na Slovensku v období od 1. apríla 2021 do 30. septembra 2021. Dáta boli zbierané prostredníctvom štandardizovaného dotazníku KDQOL-SF TM 1.3 (Hais et al. 1997) a hodnotené podľa manuálu určenému k danému dotazníku. Výsledky: Na základe výsledkov tohto výskumu bolo zistené, že vyššiu kvalitu života majú dialyzovaní seniori v doménach spokojnosť so starostlivosťou, kognitívne funkcie a kvalita sociálnych interakcií. Najnižšia kvalita života bola zistená v doménach bremeno ochorenia obličiek, celkové zdravie, obmedzenie fyzických rolí, energia a únava. Nižšie skóre dosiahli aj domény následky ochorenia obličiek, spánok, fyzické funkcie, bolesť a spoločenské fungovanie. Z výsledkov výskumu ďalej vyplýva, že kvalita života dialyzovaných seniorov nie je ovplyvňovaná dĺžkou dialyzačnej terapie ani pohlavím. Dĺžka dialyzačnej terapie nemá vplyv na výskyt bolestí ani výšku frustrácie u dialyzovaných seniorov. Bola zistená významná závislosť medzi kvalitou spánku a dĺžkou dialyzačnej terapie a medzi vekom a problémami s cievnym prístupom u dialyzovaných seniorov. Záver: Dialyzačná terapia výrazne vplýva na celkovú kvalitu života dialyzovaných seniorov. Preto je veľmi dôležité, aby u týchto pacientov bola pravidelne monitorovaná kvalita života, poskytovaná vysoko individualizovaná starostlivosť, edukácia a podpora.
